DOT Certified Driver Record Requests


All SPD Staff Attorneys and Private Bar appointed by the Public Defender's Office that submit requests through the portal linked at the bottom on this page can receive a certified copy of a Wisconsin driver record at no cost.

Requests for certified driver records made outside of this portal could be subject to a cost.

  • Requests for certified driver records made through this portal will be submitted to the DOT daily at 5:00PM

  • Your request will be processed two weeks prior to the court date. If a court date is not indicated the record will be provided two weeks from the date your request is received.

  • Certified copies of driver records are returned via U.S. Mail, unless the requestor's email address is entered, in which case it will be sent via email as an attachment.

  • The portal form will not process your request unless data is entered into all required fields.

  • Checks will be made on some fields to ensure valid data
